DEFINITION
Priapism erection is painful and settled and are not
associated with arousal and sexual gratification.
CAUSE
In adults, the cause of priapism can be known can not.
Priapism can occur in all age groups, including newborns, for example as a result
of:
-Impaired blood flow to the penis, such as leukemia, sickle
cell disease, or blood clot
-Trauma to the penis or the area under the penis
-Injury to the nerve
-Use of certain drugs, such as cocaine, a drug to treat
impotence, marijuana, or anti-psychosis particular type.
-Cancer of the penis or impede the flow of blood from the
penis
-Genital infections
-Abnormalities in the blood vessels or nerves in the
erectile tissue
SYMPTOMS
Symptoms include erection with pain that occurs without
sexual stimulation and lasts for 4 hours or more.
DIAGNOSIS
Diagnosis is based on symptoms exist and examination
results.
TREATMENT
Ischemic priapism
Ischemic priapism caused by blood can not flow out of the
penis. This is a serious condition that requires immediate treatment. Handling
is done usually done in combination with removing the blood from the penis and
use of drugs.
Aspiration
Once the penis is numbed using a local anesthetic, then the
excess blood removed from the penis using a small needle and syringe. This
method will eliminate pain, less blood oxygen content and stop the erection.
This method can also be repeated until the erection stops.
Treatment
Treatment with alpha-adrenergic sympathomimetic drugs that
are injected into the spongy tissue of the penis. The drug can shrink blood
vessels that carry blood into the penis so that blood vessels that drain blood
out of the penis will open and improve blood flow out of the penis. There is a
risk of side effects from this treatment, such as headaches, dizziness and high
blood pressure, especially if you have already have hypertension or heart
disease. Examples of alpha-adrenergic sympathomimetic drugs are Phenylephrine
and Ephedrine
Surgery
If other treatments are not successful then the surgeon can
install tools that will change the blood flow through the penis so that blood
can be normal.
Adjunctive therapy
If you're having sickle cell anemia, it can be given a
useful adjunct therapy to overcome episodes associated with the disease. Such
as the administration of supplemental oxygen or IV solutions in order to keep
hydrated.
Non-Ischemic Priapism
Non-ischemic priapism can often resolve on their own without
any treatment. Then, because there is no risk of damage to the penis, the
doctor may use a "wait and see". Put ice and put pressure on the area
of the perineum (the area between the base of the penis and anus) can also
help stop erections. Surgery can also be done in some cases, where the surgeon
will insert objects that would block blood flow to the penis. Over time, the
